ESR1 (estrogen receptor alpha) and ESR2 (estrogen receptor beta) translate circulating estrogen into tissue-level effects. The two receptor subtypes have different distributions and different functions — ESR1 dominates in breast and uterine tissue, ESR2 in bone, cardiovascular, and immune tissue. Variants in each shape how strongly tissues respond to whatever estrogen is present.
Carriers of ESR1 reduced-sensitivity variants are associated with weaker estrogen-protective effects on bone, cardiovascular health, and mood — and may experience perimenopause and menopause symptoms more strongly at the same hormone levels.
